Dr S A Aziz Noordin is a Family Physician based at Tampines Family Medicine Clinic, with a strong commitment to integrated, community-based care. Since 2019, he has been actively involved in the shared care geriatric programme in collaboration with Changi General Hospital (CGH), supporting older adults through team-based, person-centred approaches.

He holds a Bachelor of Medicine and Bachelor of Surgery (MBBS), Graduate Diplomas in Family Medicine and Occupational Medicine from the National University of Singapore, and a Diploma in Practical Dermatology from Cardiff University.

Dr Aziz’s clinical interests include mental health advocacy, dermatological conditions, minor surgical procedures, and the management of complex chronic conditions in primary care. He also serves as a Sub-lead in the Class Primary Care Network (PCN) and is an active member of the GP+ Cooperative, where he contributes to professional development and system improvement efforts across the primary care landscape.
